What it is

The Electrolytes (Sodium, Potassium, Chloride), 24-Hour Urine test measures the amount of these essential electrolytes excreted over a full day. Sodium, potassium, and chloride are vital minerals that regulate fluid balance, nerve function, muscle contraction, and acid–base equilibrium. By collecting urine over 24 hours, this test provides an accurate picture of electrolyte handling by the kidneys.

Uses

This test is used to evaluate kidney function, hydration status, blood pressure regulation, and metabolic disorders. It helps diagnose conditions such as chronic kidney disease, adrenal gland dysfunction, hypertension, metabolic acidosis or alkalosis, and electrolyte imbalance caused by diuretic use. It is also used to monitor patients receiving fluid and electrolyte therapy.

Symptoms That May Lead to the Test

Doctors may order this test if you experience symptoms such as frequent muscle cramps, weakness, irregular heartbeat, persistent vomiting, diarrhea, dehydration, swelling, or unexplained fatigue. It may also be requested in cases of resistant hypertension or abnormal blood electrolyte levels.

Abnormal Results

High Sodium (Urine): May indicate excessive salt intake, diuretic use, or kidney disorders that impair sodium retention.

Low Sodium (Urine): Seen in dehydration, heart failure, cirrhosis, nephrotic syndrome, or inappropriate ADH secretion (SIADH).

High Potassium (Urine): Can occur with adrenal hormone imbalance, tubular kidney disorders, or diuretic therapy.

Low Potassium (Urine): May be due to low dietary intake, diarrhea, vomiting, or kidney conditions that conserve potassium.

Chloride Abnormalities: Help identify causes of acid–base imbalances, such as metabolic alkalosis (low chloride excretion) or renal tubular acidosis (high chloride excretion).

Risks

The test requires a 24-hour urine collection, which can be inconvenient but poses no direct physical risk. Accuracy depends on proper sample collection throughout the entire 24-hour period.
