What it is

The CPK BB Fraction, Serum test measures the level of the BB isoenzyme of creatine phosphokinase (CPK) in the blood. CPK is an enzyme found in various tissues, and the BB fraction is primarily located in the brain and smooth muscles. Elevated levels of CPK-BB are rare in healthy individuals and usually point to neurological or certain smooth muscle-related conditions.

Uses

This test is used to detect and evaluate conditions involving brain tissue injury or disease, such as stroke, brain trauma, or certain brain tumors. It may also help assess damage to smooth muscles in organs like the gastrointestinal or genitourinary tract. Although less commonly used today due to more advanced imaging and biomarkers, it can still provide supportive diagnostic information.

Symptoms That May Lead to the Test

Your doctor may recommend this test if you have symptoms suggestive of neurological damage or smooth muscle involvement, such as severe headache, sudden weakness, confusion, seizures, loss of consciousness, or unexplained gastrointestinal or urinary symptoms. It may also be considered after head trauma or suspected brain injury.

Abnormal Results

High CPK-BB Levels: May indicate brain injury, stroke, intracranial hemorrhage, brain tumors, or damage to smooth muscle tissue. It may also be elevated in some cancers such as prostate or lung carcinoma.

Normal or Undetectable Levels: Considered normal, as CPK-BB is usually not present in measurable amounts in the blood of healthy individuals.

Risks

The test involves a simple blood draw, with minimal risks including mild pain, bruising, or dizziness at the site of collection. These effects are temporary and resolve quickly.
