Creatinine, Random Urine

What it is

The Creatinine, Random Urine test measures the level of creatinine in a single (spot) urine sample. Creatinine is a waste product generated from normal muscle metabolism and excreted by the kidneys. Since urine creatinine concentration can vary depending on hydration, this test is often used to normalize results of other urine tests (such as protein or microalbumin) by calculating a ratio.

Uses

This test is mainly used as part of kidney function assessment. It is often performed along with urine protein or albumin tests to calculate protein-to-creatinine or albumin-to-creatinine ratios, which are useful in screening for early kidney disease, especially in people with diabetes or hypertension. It may also be used as a quick check on kidney excretion ability when 24-hour urine collection is not practical.

Symptoms That May Lead to the Test

Your doctor may order this test if you have signs of kidney problems such as swelling in the legs, changes in urine output, foamy urine, high blood pressure, unexplained fatigue, or abnormal blood creatinine levels. It is also used in patients with diabetes or cardiovascular disease to detect early kidney damage.

Abnormal Results

Low Urine Creatinine: May suggest reduced kidney function, kidney disease, severe muscle loss, or overly dilute urine due to high fluid intake.

High Urine Creatinine: May be seen with high muscle mass, dehydration, or concentrated urine. It can also vary based on recent diet and exercise.

Risks

The test requires only a random urine sample, making it completely non-invasive and safe. The only limitation is that results can be influenced by hydration status and muscle mass.
