Free Thyroxine (FT4), Serum
What it is
The Free Thyroxine (FT4), Serum test measures the level of unbound (free) thyroxine hormone in the blood. FT4 is one of the key thyroid hormones produced by the thyroid gland and plays a crucial role in regulating metabolism, energy balance, growth, and development. Unlike total T4, which measures both bound and unbound hormone, FT4 reflects the biologically active hormone available to the body’s tissues.
Uses
This test is used to assess thyroid gland function and is often performed alongside TSH (Thyroid Stimulating Hormone) and sometimes Free T3. It helps in:
- Diagnosing hypothyroidism (underactive thyroid) or hyperthyroidism (overactive thyroid).
- Monitoring thyroid disorders and treatment response.
- Evaluating pituitary gland function when thyroid abnormalities are suspected.
- Assessing thyroid status in pregnancy or when protein-binding abnormalities affect total T4 measurement.
Symptoms That May Lead to the Test
Doctors may recommend the FT4 test if you present with symptoms of thyroid imbalance:
- Hypothyroidism: Fatigue, weight gain, dry skin, constipation, hair loss, cold intolerance, depression.
- Hyperthyroidism: Unexplained weight loss, palpitations, anxiety, sweating, tremors, heat intolerance, frequent bowel movements.
Abnormal Results
High FT4 Levels: May indicate hyperthyroidism, thyroid nodules, Graves’ disease, thyroiditis, or excess thyroid medication.
Low FT4 Levels: May suggest hypothyroidism, Hashimoto’s thyroiditis, iodine deficiency, or pituitary gland dysfunction.
Interpretation is usually done in combination with TSH results for a clearer picture of thyroid health.
Risks
The FT4 test requires a simple blood sample. Risks are minimal and may include mild pain, bruising, or dizziness at the puncture site. These side effects are temporary and uncommon.
